Your Appointment
Let’s make the most of your visit. Before your first appointment, please have your referring physician forward your medical records. Try to arrive 30 minutes before your first appointment, so we can verify your information. For future visits, arrive 15 minutes early.
What to Bring to Your Appointment
To each visit, please bring:
- A current, complete list of your medications
- Any applicable copay or co-insurance amounts
- Your insurance card

Prescription Refills
There are a few ways you can request a prescription refill:
- At your appointment - Tell your doctor if you’ll need a refill before your next visit.
- From the patient portal - Log in to myhealth patient portal. Under My Medications, select Renew to request a refill.
- Through your pharmacy or mail-order service - Call your pharmacy or mail-order service for refills, and they will contact us for approval.
For any refills, be sure to request them several days before you run out of medication. We respond to refill requests during business hours, so it’s helpful to plan ahead.
After-Hours Calls & Emergencies
For urgent medical care after business hours, call 816.221.6750 to reach our on-call doctor. They can help with health questions and concerns. Note that we do not call in medication refills on evenings and weekends.

Patient Registration & Forms
To reduce the time spent at our office, print and fill out any necessary forms in advance. Bring them with you to your appointment.
Tests & Procedures
Your cardiologist may recommend diagnostic tests to help assess your health and treatment options. You and your referring physician will receive your test results as soon as they’re available.
